Appointment Requests

This is a convenient form to allow you to request an appointment time.  I will contact you to confirm that your appointment has been booked to your satisfaction.  It is my goal to respond to you within 2 hours from the time your request was submitted during regular business hours.  My typical schedule will be as follows Tues 9-5, Wed 11-7, Thur, Fri, 9-5, Sat 9-4.  I'm flexible and willing to accommodate your request, so please do not hesitate to ask if you need an appointment outside of these times.  I appreciate your business and as always look forward to seeing you.
